Introduction: Presentation of our experience in Transoral Laser Microsurgery as a treatment modality on selected cases of Pyriform sinus cancer, and the outcomes of the application of this surgical technique. Estimate of effectiveness of Transoral Laser Microsurgery in the pyriform sinus cancer, as only-treatment or in combined therapy (Radio-, Chemo- therapy).

Methods: The study concerns 42 strictly selected patients, with pyriform sinus cancer T1, T2, T3, in time period 1995–2005. After the preoperative evaluation, we decided that T.L.M. could be applied. Mean age was 45–76 years old. In the 67% of the patiens postoperative radiotherapy, chemotherapy was applied. Neck Dissecton (selective or radical) was performed in all of the patients. Analysis of the results was also applied.

Results: Five years mean follow up. In 10% revision T.L.M. was applied and in 5% total larygectomy was performed. Achievement of high rate of disease free patients in a five year period. Evaluation of Transoral Laser Microsurgery as a minimally invasive technique.

Conclusion: After an equitable choice of patients, the application of Transoral Laser Microsurgery constitutes a modern and effective surgical technique, with low percentage of complications.
