Introduction: Mesh fixation in laparoscopic hernia repair is an evident problem. It is assumed that the stability of fixation is influenced by the application angle and the application pressure. In current experimental studies this aspect is not regarded. In clinical practice an ideal application angle of 90° is often not possible.
Materials and methods: Pretests will show which application angles are to be expected in clinical practice and thus be simulated. For the measurement of tensile strength a biologic model of porcine abdominal wall is developed. To ensure a standardized application of the tacks the applicator will be fixed in a steel frame. Different tacks will be applied in defined angles and pressures. Tensile strength is measured by a mechanical traction device (Mini-Zwick, Zwick / Roell, Ulm Germany) with a continuous tension on the mesh to evaluate the stability of the applied tacks.
Results: Our developed model allows a realistic simulation of force effect to tacks applied in various angles. For the first time it is possible to show the influence of mesh fixation angle and pressure.
Conclusion: This experimental study of factors influencing the stability of mesh fixation in laparoscopic ventral hernia repair can lead to evidence based and standardized application of tacks for safer results in laparoscopic hernia surgery.
