Experience of the development and implementation of medical and technological documents for the treatment of glaucoma on the principles of evidence-based medicine

Background: Primary open-angle glaucoma (POAG) is the most common form of glaucoma among the population of Ukraine, so it requires standardization of care on the basis of evidence.
Objective: Information search for high-quality clinical guidelines (CG) and adaptation of them to Ukrainian health care system
Methods: Search for CG was conducted in the G-I-N library and other sources. Clinical guideline “Glaucoma. Diagnosis and management of chronic open-angle glaucoma and ocular hypertension”, NICE, April, 2009, was taken as a prototype. Adaptation of clinical guidelines (CG) and development of unified clinical protocols of medical care was conducted by multidisciplinary working group comprised interested professionals of primary and secondary health care, health care managers, and patients. A public discussion of draft documents was conducted.
Results: A CG was adapted and unified clinical protocol for the treatment of POAG was developed for primary and secondary health care. The documents were approved by the Ministry of Health of Ukraine in November 2011. The reappraisal of the documents is planned in 2014. Adapted CG is available in the G-I-N Library.
Discussion: Implementation of adapted CG and protocols for the treatment of POAG in clinical practice is performed through the development of local protocols and patient clinical pathway on their basis. Monitoring the use of the documents will be carried out using health care quality indicators.
Conclusions for developers and users: Adaptation of clinical guidelines and development on its basis of clinical protocol allows choosing the best methods for providing medical care in the limited resources of public health care system.
